It’s said that the name comes from the knot that carriage drivers used to tie the reins on their four horse carriages, hence; the four-in-hand. Supposedly, the name was later used for a gentleman’s club in London whose members wore this knot and subsequently set the trend for the four-in-hand becoming one of the most versatile and widely used knots in the western world.
